Immunisation Programme for students in Y1-Y6

Posted: May 5, 2017

The Department of Health will visit our school on Thursday 1 June to give vaccinations to Y1 and Y6 students. Y1 students will receive Combined Diphtheria, Tetanus, acellular Pertussis and Inactivated Polio Vaccine (DTaP-IPV). Y6 students will receive Combined Diphtheria, Tetanus, acellular Pertussis (reduced dose) and Inactivated Polio Vaccine (dTap-IPV).

The Department of Health will also be checking students’ immunisation records for history of BCG vaccination. Those under 15 years without a history of BCG vaccine will be referred to the Chest Clinic of the Department of Health for further management.

The immunisation forms will be sent home next week. Please complete and return the following items before Friday 26 May:

  1. Student’s original immunisation records for checking and recording purposes. English translation is needed if the record is in another language.
  2. The signed consent or refusal form.

Please be reminded that the Department of Health requests a response and the immunisation record even if your child does not participate. The immunisation records will be returned to you on or before Tuesday 6 June.
