The Department of Health will visit our school on Monday 30 November 2015 to provide vaccinations for Y1 and Y6 students. Students whose parents have consented will receive the Measles Mumps and Rubella (MMR) vaccine. Y6 students will also receive the Hepatitis B vaccine in addition to MMR.
Please complete and return the following items before Thursday 26 November 2015:
- Student’s original immunisation records for checking and recording purposes. English translation is required if the record is in other languages.
- The signed consent or refusal form.
Please be reminded that the immunisation records and a response are still required even if your child does not participate. The immunisation records will be returned to you on or before Friday 4 December 2015.
