Immunisation Programme for Year 1 & 6 Students

The Department of Health will visit our school on Friday 22 April 2016 to give vaccinations to Year 1 and Year 6 students. Year 1 students will receive Combined Diphtheria, Tetanus, acellular Pertussis and Inactivated Polio Vaccine (DTaP-IPV). Year 6 students will receive Combined Diphtheria, Tetanus, acellular Pertussis (reduced dose) and Inactivated Polio Vaccine (dTap-IPV).

The Department of Health will also be checking students’ immunisation records for history of BCG vaccination. Those under 15 years without a history of BCG vaccine will be referred to the Chest Clinic of the Department of Health for further management.

Please complete and return the following items before Tuesday 19 April:

  • Student’s original immunisation records for checking and recording purposes. English translation is needed if the record is in another language.
  • The signed consent or refusal form.

Please be reminded that the Department of Health requests a response and the immunisation record even if your child does not participate. The immunisation records will be returned to you on or before Wednesday 27 April.
