Forms regarding Y1&6 student immunisations have been distributed. The Department of Health will visit DC on Tuesday 18 November. Year 1 and Year 6 students will receive Measles, Mumps and Rubella (MMR). Year 6 students will also receive Hepatitis B immunisation.
Please complete and return the following items before Wednesday 12 November 2014:
1. Student’s original immunisation records for checking and recording purposes. English translation is needed if the record is in other languages.
2. The signed consent or refusal form.
Please note a response and immunisation records are still required even if your child does not participate. The immunisation records will be returned to you on or before Thursday 20 November 2014.
